No but I have been in a campaign where that did happen and the DM had to pull a major fiat to be able to justify us moving on given he intended us to destroy the intentionally low skill DCs.
It was Star Wars Saga Edition set in the Old Republic Era. Basically my Sith was trying to sneak into an air vent with an Imperial Agent and they failed to disable the fane, then the agent failed to hack it, then we failed on disabling it by dropping a rock in there using the force. THEN my Sith, getting frustrated blew the whole thing up due to a critical success in shorting out the electronics with Force Lightning.
This was thile the rest of the party was failing terribly at convincing a Jedi and squad of Republic Commandos that no we certainly aren't Imperials trying to sneak into their base to assassinate a Sith that turned to the Jedi... I think we havewaved it at everyone failing horribly at putting on Coruscant accents even though one character in the party was a fallen Jedi...
Better: Fighting/Flying. It's called Hawlucha and you get get it in the same zone you can catch a wild Eevee for even more awesomeness. I call mine El Santo and it's my new favourite Pokemon.
I already have a pretty good setup for my Hawlucha online. Its gotten plenty of 3-0 and 6-0 Sweeps on Wi-Fi:

Adamant or Jolly Nature (Adamant lets you hit harder, Jolly gives you more speed and makes Revenge killing much harder)

- Encore​/Roost/U-Turn (Encore is my personal preference, but Roost is nice for dealing with Stealth Rock damage and general staying power, and U-Turn is always a great scouting move)
Lots of Competative Pokemon Jargon inside
This set aims to set up a Swords Dance by either forcing a switch (Flying and Fighting STAB hits the entire metagame for at least Neutral damage, so good luck safely switching in) or using a fast Encore to force a Pokemon into an undesirable move, then sweep using Hawlucha's high base power STABs great Attack and Speed stats.
After Swords Dance, Flying Gem Acrobatics will probably OHKO anything that isn't physically bulky (even Hippowdon will take 64% Minimum Damage from a +2 Flying Gem Acrobatics hit. And most things that resist Acrobatics? Your Rock and Ice types? They just happen to be weak to Hi Jump Kick with is 130 Power, and also gains STAb so Hawlucha can easily put a dent in anything not Aegislash (which basically hard counters Hawlucha anyway).
As added awesome to using Flying Gem Acrobatics, Hawlucha has a good ability in Unburden that doubles its already great Speed stat after its Flying Gem has been used up, making Hawlucha nearly impossible to revenge kill without a powerful priority move once it gets going. Only Jolteon has a chance at outspeeding it before a boost, after the speed boost, you're stuck with Ice Shard (Weavile is x4 Weak to Fighting and Mamoswine is weak to Fighting, both are frail as hell), or putting in a Scarfer.
