Medusa heads are predictable and only appear from the sides of the stage. Zelda II had medusa-type enemies as well, but I don't take offense to those; I'm thinking specifically of the bridge sequences where enemies would pop up from under you Cheep-Cheep style. You can get by unscathed most of the time, but you'll probably fall at least once throughout the game. And at that point you might as well kill yourself 2 more times and spare yourself the Game Over in a dungeon. It's not really a recurring issue, but it is some unwelcome icing on their sadism cake.
The "go back to the overworld" thing really ruins it for me. I think the game was pretty decent otherwise.
I find it difficult to reconcile your criticism of one-shot pit deaths in Zelda II (which were rare) versus the many, many you could get in Castlevania (see: endless medusa heads over chasms).
