By small change, I'm talking something that would require a stupidly small amount of effort on the part of a developer. Something one employee could do in a day, and then put in the product.
Personally, I'd have to go with reversible covers. Keep the 'technical'/'shelf' cover on them in the store, and then have something more artistic on the other side of the cover sheet. I've had a few DVDs that did this (although it was still a regular sleeve on each side, with the covers being based off different posters for the movie); it's an extremely small change that makes things that much cooler.
Second to that, better disk artwork. Almost every game has some kind of logo, circular design, seal, etc., use it! Most disks I see nowadays are incredibly bland. It might not matter much, but if you shun cases and keep your entire collection in a disk holder, it's a huge difference.
