So, since nobody else has seemed to bring this up, are there any clues as to what exactly one must do to achieve a successful campaign evaluation?
Right now I have a Mythril Star ∮∮∮, and I've been getting successful evaluations every time, mostly by just doing campaign ops (of which I have only failed one, a Supply Manufacture one, because I slipped and hit the wrong option). However, I just used all the credits I had, and I'm still not passing my evaluation (though, he's not directly taking away my current one, either).
I remember reading at one point that it may be directly linked to how many notes you have (I have just under 45k, 44750 to be exact). It's certainly worth looking into, and meanwhile I'll be doing some campaign battles to try to level my pup and get my next badge.

Note: It takes exactly 5000 experience/limit point to earn a new promotion through campaign battles. (Verification -desperately- required, promotions have occured at less than half that, and it seems more likely to rely on Allied Notes acquisition than EXP)
Note to the note: It took around 23k Limits through campaign battle to be allowed to rank up after 27 days of inactivity on FF. I was checking every 2k for a rank up.
