Help!, how do you link Gamertag to POL ID on the Xbox 360?

I have an account from the PC version, but when I try to log it in the xbox 360 it says "Gamertag is not linked to the POL ID"

I even made a new Gamertag and it still says not linked..

Error Code- POL-5314 is what comes up when it says the link page.

On Xbox 360 it says:

To log in to PlayOnline, you will need a linked PlayOnline ID and Gamer profile and you will need to be signed in to Xbox Live.
