Do I have to download to use the buddy pass or can I use the cd?

if you already have the CDs, then yes, you can install off that.

Is this buddy pass available only for the US market or it works with european copies of the game too? I would like to try FFXI

If youve got an american buddy pass I would imagine you will need the american game to use it and vice versa.
